Our hospital-to-home service combines non-clinical help such as meals, light home tasks, companionship, and reminders. Keep medical instructions and treatment questions with the healthcare team.
You can ask before the discharge date is final. A requested start date remains subject to availability and confirmation; do not build a discharge plan around an unconfirmed visit.

- Meals and home help — Meals cooked in the home, light housekeeping, and laundry, so the practical side of the week stops piling up.
- Appointments and errands — A steady companion for appointments, shopping, and errands outside the home, within what we can actually arrange.
Agree on the plan before visits begin
Tell us the support needed, location, preferred visit times, and any practical limits in the home. Your concierge will discuss what can be arranged and provide a clear quote before you confirm. A request does not guarantee a caregiver, start date, or particular schedule.
Vicaria Care arranges non-clinical daily support. Nursing, wound care, medication administration, and rehabilitation belong with appropriately licensed providers. Medication reminders do not include giving medication. Caregiver-provided transportation, overnight care, and round-the-clock coverage are not promised.
